leigh-m3: MASS-1: Refactored getCurrentSegments() to treat currentSegments more like the queue it is

leigh-m3: MASS-1 reverted size of playlist to 5, and moved the currentSegments queue to the beginning of processing.

leigh-m3: Removed the filename translation code, removed the ring buffer, now that there is proper disco support. Added URL encoding to solve issues with weird filenames and the front-end proxies.

leigh-m3: Introduced #EXT-X-DISCONTINUITY-SEQUENCE and #EXT-X-DISCONTINUITY support. Currently is a memory leak.
