OSD-replication:

- repaired SimpleStrategy, so it could be used again
- some performance optimizations (dynamic creation of entries in lists/maps, first if they are required)
- replication flags are (partly) supported by replication, RAF and tools
- holes could be (easily) determined through checking if last used replica is a full replica
